Problems of anticoagulant therapy in patients with prosthetic heart valves exist to this day, as there are no national recommendations and there is no consensus on the strategy of anticoagulant therapy in this category of patients. The problems are associated not only with the difference in the drug response of warfarin as a result of the influence of external, internal and genetic factors, but also with the thrombogenicity of prosthetic heart valves. The use of pharmacogenetic testing to detect sensitivity to warfarin, the use of portable devices to determine the threshold level for the onset of coagulation cannot solve the problems posed without increasing adherence to treatment and awareness of anticoagulant therapy not only for doctors, but also for patients.
