Two-component tariff of insurance premiums for CMI as a basis for innovations in payment for medical services

In the article, the authors raise the problem of paying for medical care provided to citizens. In particular, the problem of estimating the cost of these expenses is noted due to a number of factors (the presence of concomitant diseases, intolerance to certain medical procedures or medications, refusal to follow doctor’s orders, incurability of certain diseases, seasonality of demand for certain medical services, etc.). Particular attention is paid to the problems of using clinical and statistical groups to pay for medical care. To solve existing problems, the authors proposed using a two-component tariff of insurance premiums for compulsory health insurance as the basis for innovations in payment for medical services, separately taking into account fixed and variable costs. The practical implementation of the author’s recommendations will not only ensure the formation of a stable, reliable basis for modernizing the payment system for medical care, but will also have a positive impact on improving the quality of its provision to citizens. The results of the study can be used in the practical activities of the Government of the Russian Federation, the Federal Compulsory Medical Insurance Fund, the Ministry of Health of the Russian Federation, the Federal Assembly of the Russian Federation as part of modernizing approaches to financial support for the healthcare system.
