This paper examines the transformation of the Russian pharmaceutical market in the context of the national policy of import substitution of medicines (generics, original medicines). A structural analysis of the Russian pharmaceutical market for 8 years was carried out in the context of import substitution. The current trends in the localization of pharmaceutical manufacturing are analyzed. Values of sales concentration by groups of the largest manufacturers are obtained. Consumers of inexpensive medicines become victims of high prices for original medicines. Russia has achieved some success in the localization of pharmaceutical production on its territory over the past 8 years. 5 out of 6 drug packages sold in the Russian Federation are generic drugs. A large number of competing manufacturers operate on the Russian market. At the same time, Russia remains dependent on imports of original drugs and pharmaceutical substances. Further development of the pharmaceutical market depends on the development of innovative medicines, which requires time and venture investments. The success of the state in the development of innovative infrastructure and human resources will determine who will be the beneficiary of royalties, Russian or foreign patent holders.
