The results of surgical treatment of 90 patients with acquired shortened oesophagus were analyzed and exposed to thorough investigations, taking into account the main pathogenetic, pathophysiological factors and mechanisms of development of reflux-oesophagitis and hiatal hernias. Modern technologies used in treating these patients were observed with a critical point of view. On the basis of personal experience and obtained results it was settled that at 1st stage shortening of the oesophagus it was logical to use the fundoplication modification by A.F. Chernousov. At 2nd stage shortening of the oesophagus the valvular gastroplication was indicated. It was shown that at formulating the tactics of treatment of the patients with shortened oesophagus and reflux oesophagitis it was necessary to take into account the stage of shortening, severity of inflammatory and sclerotic changes in the oesophagus, functional reserve and motility of the oesophagus.
