This article represents a new method for aortic coarctation and distal arch hypoplasia surgical treatment in newborns. The proposed original technique allows an effective off-pump correction of coarctation with enlargement of hypoplastic distal aortic arch segment and preservation of anterograde blood flow in the left subclavian artery. In Pediatric cardiac surgery center of Meshalkin institute, through years 2009-2011, 9 neonates with preductal aortic coarctation and hypoplasia of the distal segment of aortic arch had underwent the following procedure: an extended anastomosis formation, combined with distal aortic arch enlargement by reverse subclavian flap repair and subsequent implantation of the distal segment of the left subclavian artery into the left common carotid artery. The article contains a detailed description of the following surgical technique.
