Hyperthermic isolated pelvic chemoperfusion

First experience of hyperthermic isolated pelvic perfusion (IPP) in a patient with tumor of lower and middle third of vagina cT4N+M0 complicated by recurrent bleeding is presented in this article. Vascular isolation was achieved by occlusion of aorta and inferior vena cava in infrarenal region under fluoroscopic control using occluding catheters and pneumatic tourniquets (2 - on the upper hips and 1 - on the paraumbilical region). Cisplatin 250 mg/m2 was used as an antitumor agent. IPP was performed via cannulated femoral artery and vein. This technique provided highly effective isolation of the perfused region, minimal leakage of the chemotherapeutic drug to systemic circulation, absence of systemic side-effects and postoperative morbidity. Clinical result that was achieved allows to consider procedure of hyperthermic IPP as an effective neoadjuvant antitumor modality in managing patients with locally advanced pelvic malignancies.
