Integrated approach to prediction of long-term outcomes of conservative treatment for benign prostatic hyperplasia

In this study, we analyzed the role of several etiopathogenetic and clinical factors important for the prediction of conservative treatment outcomes in patients with benign prostatic hyperplasia (BPH). The study included 849 patients who received conservative treatment (n = 441) or underwent surgery (n = 408) for BPH between 2012 and 2015. Some clinical features of BPH were found to increase the risk of conservative treatment failure. The following factors were associated with poorer outcomes of conservative therapy: middle lobe of the prostate affected (according to TRUS), history of acute urinary retention, prostate volume growth rate (according to TRUS), prostate biopsy, no therapy with 5-alpha reductase inhibitors, use of minimally invasive treatment for BPH in the past. Correlation and regression analysis revealed a number of the most significant independent factors that were later used for the development of a mathematical model for predicting the outcome of conservative therapy for BPH and a software to assess the prognosis.
