A new technique of triple pelvic osteotomy with simultaneous reconstruction of the hip pelvic and femoral components

A new method of triple osteotomy of the pelvis has been developed for the treatment of children with congenital hip joint pathology. The intersection of the ischium is carried out from the inter-cleft access after the osteotomy of the femur. It does not require additional access to it, which reduces the traumatic nature of the intervention. The lone bones intersect at the base, the iliac is transverse or semicircular above the inferior anterior iliac spine. A bone fragment resected in osteotomy of the femur is used to fill the defect and fix the mobilized acetabular fragment in the nadacetabular region. The proposed method was used in 13 children aged from 9 to 14 years on 14 joints with pathology of dysplastic genesis (in eight patients on nine joints), with osteomyelitis consequences (2), aseptic necrosis outcome after conservative treatment of congenital dislocation of the hip (1), disease Perthes (1), paralytic dislocation of the femur (1). The follow-up period after the operation was from six months to six years. In studying the results obtained, in all cases positive clinical and roentgenologic dynamics were noted, congruence and stability in the hip joint were achieved, in no case there was damage to the vascular-neural formations, full bone coverage of the femoral head was achieved. The proposed method allows to reduce technical difficulties and secure surgical intervention in one-stage reconstruction of the pelvic and femoral components.
