Complexities of differential diagnosis of hypokalemia syndrome. Clinical case

Differential diagnostics of hypokalemia covers a wide range of diseases, including poorly recognized kidney pathology. To facilitate diagnosis, it is necessary to determine the presence or absence of hypertension in patients. With normal blood pressure, it is necessary to suspect Bartter syndrome and Gitelman syndrome. A clinical case of patient P., 24 years old, with hypokalemia syndrome is described. The stages of diagnostic search within the framework of hypokalemia syndrome are outlined. Differential diagnosis was made between Bartter syndrome, Gitelman syndrome, Liddle syndrome, primary hyperaldosteronism, Itsenko-Cushing syndrome. The diagnosis of the classic form of Bartter syndrome was established. Treatment with indomethacin 25 mg 2 times a day after meals was recommended. Genetic studies for Bartter and Gitelman syndromes can be the most convincing tool for diagnosis. However, we did not conduct genetic research due to lack of technical capability.
