The assessment of peripheral hemodynamics by methods magnetic resonance imaging and angiography in obliterating atherosclerosis of the lower extremities arteries

Purpose of the study was to define potential of contrast magnetic resonance angiography of lower extremities and magnetic resonance imaging of muscles of lower legs in the evaluation of peripheral hemodynamics in the arteries of the lower extremities in patients with obliterative atherosclerosis of the lower extremities’ arteries. Contrast magnetic resonance angiography is featured by a high level of information capacity in diagnosis of obliterating lesions in lower extremities. Sensitivity and specificity of contrast magnetic resonance angiography were 95, 8% and 96, 8% (for aorta-femoral and femoropopliteal segments) and 81.5%, 96.3% - for lower legs arteries, respectively. The existence of a statistically significant relationship between preoperative indicators of the severity of degenerative changes of leg muscles on magnetic resonance imaging, intraoperative histological examination of biopsy material of leg muscles and the level of regional vascular resistance after the restoration of blood flow. Preoperative assessment of the level of regional peripheral vascular resistance would provide maximum personalized tactics surgery, complementing recent corrective hemodynamic manipulation.
