The issues of the organization of import-substituting production of high-tech medical devices, its transfer to an intensive type of development in the conditions of the requirements of ensuring a high quality of life are considered. An expert assessment of the state of the world market of medical devices on a microelectronic basis was carried out, on which the positions of Russian enterprises look quite modest, and many segments of the domestic Russian market of medical products were lost. In a country striving for technological sovereignty in critical industries, it is important to organize the production of promising products in cooperation with enterprises of friendly countries, subject to accelerated commercialization of its own targeted research and development. In the conditions of opposition to total sanctions restrictions, many import substitution programs should be implemented accordingly in order to ensure the technical, medical and drug safety of the country. However, this will require large-scale investments in research and development of promising globally competitive products, which will require long payback periods due to market limitations. The results of the study lead to the conclusion that it is necessary to organize modern production facilities for the production of high-tech medical products, using design methods of organizing work, not to the detriment of the technology already established at the enterprise. At the same time, it is necessary to strive to develop cooperative ties with firms of friendly countries for the production and sale of promising medical products, both at the research and development stage and commercial implementation, striving for payback by providing access to capacious global markets.
