Management of patients with concomitant traumas and splenic rupture

We analyzed treatment of 256 patients with concomitant traumas registered over the last 9 years. The diagnosis was based on ultrasound examination and computed tomography (CT). Forty-five patients had splenic injury; of them, 22 patients underwent splenectomy with autologous transplantation (Group 2) and 23 individuals had splenectomy without autologous transplantation (Group 1). We compared postoperative complications in the two groups. Inflammation of the surgical wound was the most common complication observed in 6 patients from Group 1 (13.3%) and 2 patients from Group 2 (4.4%). Pneumonia was diagnosed in 5 patients from Group 1 (11.1%) and 2 patients from Group 2 (4.4%); all of them received conservative therapy. Our findings suggest lower frequency of inflammatory complications in patients who underwent autologous transplantation in addition to splenectomy compared to those who had splenectomy alone.
