Primary osteosynthesis according to Ilizarov - the main factor of prevention and treatment of purulent complications of open fractures

The authors studied the character of wound healing in leg bone open shaft fractures of different severity degree. Primary osteosynthesis according to Ilizarov was performed in 170 patients, 409 patients were subjected to other methods of treatment. It was found that in open fractures of I Degree the character of wound healing didn't depend on the method of treatment. As for II Degree, when primary osteosynthesis according to Ilizarov was used the number of purulent-and-septic complications decreased about seven-fold. Despite the fact that there were no statistically significant differences in the incidence of complications between the groups for III Degree of open fractures, in most cases of this category it was not possible to use any other technique except primary osteosynthesis according to Ilizarov. Clinical cases in the work illustrate successful use of the Ilizarov fixator for the purulent complications already developed.
