The use of various methods for fixing intraocular lenses in aphakia with tamponade of the vitreous cavity with silicone oil

Various methods of fixing an intraocular lens is an urgent issue for combined pathology of the retina and aphakia. The absence of a barrier between the anterior chamber and the vitreous cavity of the eye is the main risk of silicone penetrating into the anterior chamber, which can cause severe complications. The role of the barrier can be performed by the anterior chamber intraocular lens, in the setting of which, the migration of silicone does not occur. However, this type of fixation leads to a number of severe postoperative complications. In our country, a widespread method is the use of pupil lenses in case of insolvency of the capsule-ligamentous apparatus. Closure of iridectomy with a silicone bladder can dramatically increase intraocular pressure. With iris-capsule fixation, the frequency of lens dislocation, subsequently complicated by the penetration of silicone into the anterior chamber, casts doubt on the merits of this method. The most physiological is the method of transcleral fixation. It combines the minimum number of postoperative complications, while performing the role of a barrier between the anterior chamber and the vitreous cavity. However, this method is the most difficult surgical intervention.
