The paper analyzes the main problems of medical support of the Russian regular army and the Cossacks in the Caucasian war reflected in the documents of the military administration of the XIX century. The study uses historical-systemic, historical-genetic and historical-typological methods. It is shown that the healthcare system in many respects did not meet the requirements of wartime. Despite the fact that in the beginning of XIX century in the Caucasus appeared permanent and temporary hospitals and infirmaries, and medical professionals (doctors and paramedics), conditions for treatment of the wounded and the sick remained at a fairly low level and was compounded by the shortage of medical personnel, shortage of place in medical institutions, deficit of medicaments and bandages, ineffectiveness of most treatment methods (especially in the provision of emergency medical care). By the mid-nineteenth century many hospitals and the infirmaries went into decline, and thus were closed.
