Mitral valve replacement was performed in 88 children. They were divided into three groups according to various degree of preservation of chordae tendineae and papillary muscles: total excising of subvalvular apparatus, partial chordal-sparing technique, complete preservation of chordae tendineae and papillary muscles. Patients with chordal-sparing techniques demonstrated better left ventricular function, survival rate and exercise tolerance in early and mid-term postoperative period.
