The work involves analysis of the possibilities of radionuclide research methods - ECG-synchronized single-photon emission computerized imaging and positron emission maging in assessment of the myocardium viability and differential diagnostics of the character of its affection. The literature data provide the basis for substantiation of the advantages of these kinds of the research compared to other modern methods of perfusion visualization, contractility and metabolism of the left ventricle myocardium in selecting and planning patients with coronary disease with inoperable affection of coronary vessels for operative therapy with application of alternative technologies aimed at improvement of the regional myocardial perfusion through neoangiogenesis. It has been shown that the standardized approach of complex analysis of perfusion and myocardial function forming the basis for scintigraphic images processing in ECG-synchronized single-photon emission computerized imaging allows to evaluate the dynamics of the bloodstream at the microcirculation level in the myocardial areas where «address» revascularization was performed, which allows to assess its effectiveness and to study the mechanisms of modern alternative technologies developed for treatment of patients with CHD.
