Early transosseus osteosynthesis of open shaft fractures of leg bones according to Ilizarov as a preventive factor of complications and unfavorable outcomes in patients with polytrauma

The immediate and long-term results of treatment for open shaft fractures of leg bones in 127 patients with isolated fractures and in 358 patients with polytrauma were studied. The number of complications and unfavorable outcomes were studied in groups of patients with polytrauma of various severity. On admission transosseus osteosynthesis according to Ilizarov was performed in a part of patients after initial debridement. In the others conservative methods were used for immobilization of fragments. More severe process of the healing of open leg bone fractures was noted in patients with polytrauma. Complications and unfavorable outcomes increased in number as far as polytrauma severiry became higher. In comparison with plaster cast and skeletal traction use that of early osteosynthesis according to Ilizarov reduces the number of complications and unfavorable outcomes significantly (with statistical validity), providing easier process of traumatic disease after polytrauma.
