Results and prospects of treatment non-metastatic cancer of the stomach, complicated by bleeding

We analyzed the results of treatment of 142 patients with non-metastatic gastric cancer, complicated by bleeding. As in the control and so in the main group the vast majority of gastric bleeding were observed at gastric cancer stages III-IV. The preferential localization of the tumor was noted in the body of the stomach. Most often, patients arrived with one complication - bleeding. In the 2nd group there was no unified therapeutic conception and patients were operated mainly on an emergency basis at admission or in case of recurrent bleeding. When treating patients of the 1st group was used individual medical-diagnostic tactics taking into account the degree of endoscopic hemostasis, the severity of the bleeding and the patient’s condition. The main indication for emergency operations in group II was: the continuous bleeding, the inefficiency of endoscopic hemostasis and recurrent bleeding in a hospital, in group I - besides continuous bleeding and ineffective endoscopic hemostasis was determined the risk of recurrent bleeding - at high risk of recurrent bleeding, patients were operated on urgently after a full preoperative preparation without waiting for a recurrence of bleeding. It should be noted that only in the II group performed wittingly such a non-radical operative intervention as resection of 2/3 of stomach and resection of the cardia. In addition, it should be noted that in the I group used a combined endoscopic hemostasis using argon plasma coagulation in combination with the Endoclot hemostatic system.
