Current ideas of microsurgery role in restorative and reconstructive surgery of limbs are presented on the basis of the data of literature. The review of the authors' developments is given, which are based on combining microsurgery with controlled transosseous osteosynthesis: using the effect of distraction histogenesis for filling posttraumatic defects of peripheral nerves and histogenetically substantiated ways of nerve microsurgical suture; optimization of the protocol of restorative operation and postoperative management of patients with forearm deep injuries; two-stage technique of osteosynthesis for replantations and revascularizations of hand fingers and fragments, separated by high-velocity mechanisms.
