The present work is devoted to the comparative analysis of the results of myocardialrevascularization by Off Pump techniques for Coronary Artery Bypass and extracorporealcirculation with cardioplegic cardiac arrest in patients with acute coronary syndrome. When comparing the indicated surgical techniques revealed the advantages of coronary bypass surgery on working heart, as a rare development in the postoperative period, acute heart failure, cardiac arrhythmias, neurological complications type I, acute respiratoryfailure; shortening the length of artificial lung ventilation; the reduction in the total duration ofintra-aortic balloon pump therapy in the pre - intra - and postoperative periods, the time fromthe start of the operation to arteriotomy target vessel, the duration of surgery, stay patientafter surgery in the hospital. At the same time, the frequency of use of intra-aortic balloonpump therapy during surgery and the postoperative period was slightly higher in patientsoperated by the method of "off pump”, although the length of stay of patients in cardiacintensive care after surgery in both groups were the same. The index revascularization forOff Pump Coronary Artery Bypass was lower. Finally, in-hospital mortality among patientsoperated by the method of "off pump” was significantly less.
