Treatment of leg bone defects of different origin still remains one of the most difficult parts of modern reconstructive-and-restorative surgery; and among them the subtotal tibial defects, in which the whole of shaft bone part is absent, are the most difficult for rehabilitation. The tactics of their filling in the presence of ipsilateral fibula is based mainly on its use as plasty material. However, the problems of the sequence of fibular graft joining with tibial fragmental ends, as well as technique variants for creating the contact between them remain debatable. The own experience of the authors is presented and analyzed as to successful treatment of 10 patients with subtotal leg bone defects of posttraumatic or other genesis, as a basis of which the principle of sequential distal-and-proximal tibiofibular synostosing, made mainly in a graduated way by "side-to-side" type using transosseous osteosynthesis potential has been taken
