Actuality: the separation surgery, according to the terms of their performance, can increase the efficiency and reduce the number of complications in patients. Surgical interventions for the prevention of recurrence of primary spontaneous pneumothorax (PSP) on terms not separated, although performed in different time periods. Thus, it is necessary to define the terms of surgical interventions in the PSP. Compare their influence on the early postoperative period and long-term results, as well as to identify the best. Materials and Methods: From 2005 to 2013 was carried out 91 surgical intervention aimed at relapse prevention PSP, including the apical pleurectomy and chemical pleurodesis yodopiron. Patients were divided into 3 groups. The first group of 28 patients who underwent urgent, the second group of 25 patients who underwent delayed, and the third group of 38 patients who underwent elective surgery. Comparison of groups was carried out on time intervals of the disease, early postoperative period and long-term results. For the statistical evaluation of the results of the analysis of qualitative characteristics used Fisher’s exact test. When evaluating quantitative traits analysis of variance was used. Results: The following results I, II and III groups, respectively - the length of a surgery (minutes): 63.9 ± 26.4, 61.9 ± 16.7 63.2 ± 20.3 p> 0.05; long standing drainage after surgery (days): 5,6 ± 2,75 ± 3,17,4 ± 4,3 p
