30 men with type 2 diabetes mellitus, overweight or obesity of the first degree and erectile dysfunction took thioctic acid at a dose of 600 mg in the morning on an empty stomach for 11 weeks. Androgens (transdermal testosterone 50 mg) were then added to thioctic acid once a day subcutaneously for 9 months (37 weeks). All patients took gliclazide-MV at a dose of 60-90 mg once a day. Treatment with thioctic acid at a dose of 600 mg after 11 weeks led to an reliable increase in total testosterone, a decrease in the values of sex hormone binding globulin, an improvement in the lipid spectrum, carbohydrate metabolism, and a decrease in insulin resistance. The addition of transdermal testosterone 50 mg 1 time per day to thioctic acid after 48 weeks of treatment, compared with the initial state, led to a significant decrease in sex hormone binding globulin, increased total testosterone concentration, improved glycemic control and decreased insulin resistance. Transdermal testosterone and thioctic acid have a significant positive effect on erectile dysfunction and metabolic disorders in patients with type 2 diabetes mellitus.
