Questions for drening after total hip arthroplasty

Aspiration drainage of operating wounds is a fairly common medical manipulation, the purpose of which is the removal of fluid from the operating wound (mainly blood). It is believed that aspiration drainage reduces the likelihood of development of hematomas and infections of the area of surgical intervention. In turn, aspiration of wound fluid from the surgical wound through drainage reduces the diapedesis of the wound fluid through the sewn layers of the wound and promotes its healing by the primary type. The economic feasibility of using drains is determined by the fact that they reduce the number of necessary dressings in the postoperative period. On the other hand, aspiration drainage has a number of potential drawbacks. It has been established that drainage is a risk factor for infectious complications, and the removal of drainage can be problematic in hip replacement. Repeated surgeries further increase the risk of infectious complications. Drainage is inhibited by independent hemostasis due to a constant decrease in pressure and mechanical traumatization of tissues, which leads to an increase in postoperative blood loss. The literature data were analyzed from 1988 to 2017, various aspects of this problem were examined, the relevance of the presented studies and the prospects for further study of the problem of draining the postoperative wound after primary total hip arthroplasty were determined.
