Choosig the method of operative therapy of complicated colon cancer

In the period from 1995 to 2007 the FTI «NMSC named after N.I. Pirogov» administered treatment to 226 patients for complicated colorectal cancer with the following prevailing symptoms: obstruction of the colon - I group - 97 (42,9%), inflammatory paracancerous complications with elements of abscess formation - II group - 51 (22,6%), tumor perforation with peritonitis of various degrees of dissemination -III group-43 (19%), heavy enteror-rhagia - IV group - 35 (15,5%). Application of modern survey methods, including U/S, CT, MRI, endoscopy and endovideosurgery allowed in most cases (91,4% of the patients) to di-agnose both localization and dissemination of the neoplastic process and the related complications at an early stage of the disease. The treatment results in the I group of the patients were characterized by the highest number of complications (48,8%) and fatal cases (18,6%). The best results of operative therapy of patients with complicated colorectal cancer are directly dependent on timely diagnosis of the underlying disease and its complications, reasoned surgical approach, choice of the method and technique of the operative intervention as well as on sound prevention and therapy of postoperative complications.
