Лечение пациентов с терминальной стадией артроза голеностопного сустава на сегодняшний день является актуальной и нерешенной ортопедической проблемой. Консервативное лечение пациентов с данной патологией чаще всего является неэффективным. Одним из самых популярных хирургических методов лечения таких пациентов в нашей стране и за рубежом является артродезирование голеностопного сустава. Эта операция направлена на устранение болевого синдрома, восстановление опороспособности нижней конечности и улучшение качества жизни. Для выполнения артродеза могут быть использованы различные доступы к голеностопному суставу в зависимости от типа деформации, способа фиксации, состояния мягких тканей, предпочтения хирурга и его опыта. В настоящее время разработано большое количество разнообразных хирургических опций которые могут быть использованы в ходе данного вида хирургического вмешательства: внешняя фиксация, внутренняя фиксация с использованием накостных пластин, винтов и интрамедуллярных стержней; существуют различные варианты операций отличающиеся степенью хирургической агрессии. Несмотря на большую изученность проблемы, в настоящий момент отсутствуют общепринятые алгоритмы и подходы к селекционному отбору пациентов и технике выполнения данной операции. В статье представлен анализ публикаций, описывающих различные методики выполнения артродезирования голеностопного сустава и их результаты.
