The current article describes modern methods for risk assessment of peri- and/or postoperative cardiac complications in non-cardiac surgery in elderly with known or suspected coronary artery disease. Myocardial perfusion imaging with gated single-photon emission tomography is a useful, highly informative diagnostic tool for detecting and assessing the severity of coronary artery disease among old patients undergoing non-cardiac surgery, which definitely plays an important role in making decision about further management of coronary artery disease and to prevent possible contraindications.
