Диагностика перипротезной инфекции при ревизионном эндопротезировании коленного сустава: обзор литературы

Статья посвящена одной из самых актуальных проблем современной ортопедии - диагностике перипротезной инфекции. В статье представлены современные взгляды на различные методы диагностики и их комбинации в выявлении перипротезной инфекции с акцентом на особую группу пациентов, которые нуждаются в ревизионном эндопротезировании коленного сустава по причине асептической нестабильности компонентов эндопротеза или при наличии спейсера, установленного по причине перипротезной инфекции. Цель обзора - на основании анализа литературы представить современные взгляды на диагностику перипротезной инфекции у пациентов, нуждающихся в ревизионном эндопротезировании коленного сустава. На основе анализа литературы сделаны выводы о том, что чувствительность и специфичность ряда методов диагностики значимо снижается при обследовании пациентов перед ревизионным эндопротезированием, что не позволяет своевременно дифференцировать асептическую нестабильность компонентов эндопротеза и латентную перипротезную инфекцию или оценить эффективность этапа санации перед ревизионным эндопротезированием коленного сустава. Совершенствование диагностических алгоритмов комплексного обследования пациентов, нуждающихся в ревизионной операции, вероятно позволит своевременно выявлять или исключать латентную перипротезную инфекцию и скорректировать при необходимости тактику лечения пациентов с выявленной инфекцией.
