Diagnostics, prophylaxis and treatment of intraabdominal adhesions after appendectomy

The results of surgical treatment of 240 patients with acute appendicitis are presented. In the diagnosis of acute appendicitis, clinical, laboratory (N-acetyltransferase enzyme activity, a sorbent-based test system with magnetic properties with immobilized granulated antigen from adhesive tissue), instrumental methods of investigation were used. The first (control) group included 240 (50%) patients who underwent traditional appendectomy. Since 2017, with the treatment of 240 patients with acute appendicitis (the main group), the elements of “Fast-Track” surgery have been used. All patients with a diagnosed adhesive process in the early stages of its development were treated with anti-adhesion therapy. The diagnostic value of the ultrasound examination in detecting the formed intraabdominal adhesions was lower than that of the sorbent-based test system with magnetic properties with immobilized granulated antigen from the adhesive tissue. The multimodal approach in the treatment of acute appendicitis made it possible to reduce the formation of postoperative intra-abdominal adhesions from 10.2% to 5.8%.
