Performance of indicators of hemodinamics in optimization of infusion and transfusion therapy with mass bleeding

The article deals with carried investigation noted that hemorrhage of category 1 (to 15 % of blood volume) of patients with bleeding gastro duodenal ulcer is compensated with spasm of arterial vessels in the big cycle of blood circulation and isn’t followed by evident hemodynamic disorder. In response to increasing afterload and some reducing of cardiovascular adaptability, compensatory tachycardia on background of reduced stroke volume provides normal parameters of minute cardiac spike. Compensation of bleeding up to 750 ml of patients with gastro duodenal bleeding reaches by transfusion of balanced Strefundin isotonic solution that provides normalization of hemodynamic indicators.The homeostasis of patients with ulcer gastro duodenal bleeding 750 -1500 мl is carried out by Strefundin and colloidal plasma substitute on the base of modified gelatin Helofusin in correlation 3:1, that normalizes main functional parameters of homeostasis. Introduction of developed program of infusion therapy, referred to compensation of bleeding in ulcer gastro duodenal bleeding into clinical practice, excluded fatal outcomes and the necessity to carry out surgery because of ongoing bleeding and reduced the number of complication and rate of endoscopic ulcer coagulation.
