Hemodynamic effectiveness of surgical correction of congenital mitralin sufficiency

Existing anatomically complex variants of congenital mitral valve surgical pointof view determines the need for accurate preoperative evaluation of the anatomy andfunction of the atrioventricular valves, as well as evaluating concomitant intracardiacanomalies. Carrying intraoperative assessment of the pumping function of the heart toevaluate the effectivness of the valvuloplasty and mitral valve replacement. Definition of evidence and comparative evaluation of the efficiency of reconstructivesurgery and mitral valve replacement in congenital insufficiency, depending on changes invalvular anatomy and hemodynaic overload. The analysis of changes in hemodynamic parameters and results of treatment ofvarious forms of congenital mitral valve insufficiency in 61 patients aged 4 to 38 yearsold. Hemodynamic assessment was made with cyclic analysis of cardiac activity. In patients undergoing plastic surgery and valve prosthesis with preservation ofvalve apparatus, almost always possible with minimal pharmacological support to usethe Frank-Starling law to preserve adequate blood flow and optimal contractile state,whereas when the valve prosthesis removing subvalvular structures requires the useof adequate load the volume and the use of cardiotonic agents. These positive changesare due to the fact that due the restoration of normal function of the valve by meansof entertaining the prosthesis if stored savvy structures in the cardiac cycle reappearsphase of isovolumic contraction of the left ventricle. At the beginning of systole afterthe closing of the mitral prosthesis and to contact a valve opening owing to contractionof the papillary muscle, a convergence of left ventricular apex and the mitral annulusplane that describes transformation ventricular cavity, the optimal stretching circularfibers and strong contraction of the heart.
