The influence of complex pathogenically substantiated conservative treatment including application of chondroprotectors, non-steroid anti-inflammatory preparations, calcium preparations, physical therapy procedures, exercise therapy on the indices of system immune response in 87 patients with idiopathic I-II stage osteoarthrosis of the knee and the hip has been studied. It has been demonstrated, that during treatment the main values of cellular and humoral immunity don't change significantly (CD3+, CD3+CD4+, CD3+CD8+, CD3-CD19+, CD3-CD16+CD56+, CD3+CD16+CD56+, CD3+CD25+, IgA, IgM, IgG), they are those which don't initially differ from the values of control group and don't go beyond the limits of normative values. In the group of patients with good and satisfactory results of treatment statistically significant decrease of serum IL-1β was revealed within 6-24 months, that of serum TNFα - within 12-24 months after conservative therapy beginning. In case of therapeutic effect absence (pain syndrome keeping) short-time decrease of IL-1β level was observed in 6 months, that of TNFα concentration - in 12 months after course treatment start. It is recommended to use the data obtained for evaluation of the effectiveness of therapeutic interventions in patients with early stages of large joint osteoarthrosis in outpatient conditions
