In patients with unresectable colorectal cancer metastasis to the liver locoregional chemotherapy, allowing to keep high concentration of antineoplastic drugs in tumor cells with minimal damage to healthy tissue, seems promising. The aim of the study was to evaluate in experimental conditions in laboratory animals, opportunities and safety of the isolated hepatic perfusion, to choose the optimal modes and techniques perform this procedure. The study was performed in 5 pigs with weight 25-55 kg. The original installation for the technical implementation of the hepatic perfusion was created. The method and the technique of stepwise procedure isolated hepatic perfusion in which the inflow and outflow of the perfusate is carried out through the portal vein and the inferior vena cava (the portion above the discharge of hepatic veins), respectively, and venous return is provided by the formation of additional veno-venous circuit was developed. The results of intraoperative monitoring indicators of vital functions of animals did not reveal significant abnormalities, suggesting good portability of the procedures. All the animals had surgery successful. Histological examination of liver samples taken during and after perfusion showed complete safety and vitality of the liver cells. The isolated hepatic perfusion is technically feasible, reproducible and relatively safe procedure.
