Isolated systolic hypertension and comorbidity in elderly and old men

Aim: to study the frequency and nature of comorbidity (impaired glucose tolerance, type II diabetes, kidney dysfunction, chronic renal insufficiency) with isolated systolic hypertension in elderly and old men. We examined 215 men with hypertension aged 60-88 years, of which 115 persons with isolated systolic hypertension (ISH Type I - 63, ISH Type II - 52). 100 people with systole&diastolic arterial hypertension were included into the comparison group. We assessed hemodynamic, coefficient of susceptibility of the arterial wall. ECG was recorded in 12 standard leads. We determined the echocardiographic parameters of the heart, the index, left ventricular mass and its geometrical model were calculated. The metabolic rate VLDL&cholesterol/HDL&cholesterol, the state of carbohydrate metabolism with the assessment of insulin resistance are taken into account. Attention is paid to the functional state of kidneys. The significance of the coefficient of compliance was shown for the assessment of the functional state of the vascular bed. It was noted that the progression of metabolic disorders depend on the duration of hypertension. We found more significant changes in metabolism in normal geometry of the left ventricle in patients with isolated systolic arterial hypertension. Plausible role of fasting and postprandial glucose in the relationship of the coefficient of the arterial wall compliance of the left ventricular mass in isolated systolic hypertension I type is significant.
