Economic and legal aspects of accessibility of medical care to citizens of the Russian Federation

In the Russian Federation, accessibility of medical care plays an essential role in the implementation of the principle of protecting the health of citizens. Accessibility of medical care is characterized, first of all, by equal economic and legal conditions for medical organizations when providing the same medical services. Regional tariff agreements in the field of compulsory health insurance, under such circumstances, should meet real financing needs. However, on the territory of different subjects of the Russian Federation, the constituent elements of accessibility of medical care are different. Thus, there are no uniform tariffs for medical care in different regions of the Russian Federation. The reasons for this lie in the absence in federal legislation of requirements for the development of a unified approach to the formation of tariffs; different source data and the structure of morbidity in the regions of the Russian Federation, etc. Mechanisms to increase the availability of medical care, in our opinion, will be challenging subjects of compulsory health insurance tariff agreements; consolidation at the level of the current federal legislation of uniform methods for calculating tariffs in the subjects of the Russian Federation, uniform codification, names and content of individual services; exclusion of financing of non-targeted costs of medical institutions from the costs included in the tariff for medical care.
