A case of total endoprosthetics of the hip after chronic posttraumatic osteomyelitis, developed due to the hip gun-shot shrapnel wound, is clinically analyzed. Osteomyelitis treatment has resulted in producing an extensive defect of the proximal femur (12 cm) as well as in complete absence of the hip. The authors describe active surgical tactics, consisting in total cemented endoprosthetics made with an oncological endoprosthesis of ESI firm together with osteoplasty of the acetabular roof, in limb shortening elimination at the expense of proper selection of the joint diaphyseal component. The main principles of conservative therapy in such patients are given, including intra-arterial infusion of antibiotics and other preparations, HBO, laser therapy.
