Life quality of patients who are on chronic hemodialysis and who have a permanent vascular access

Comparison of life quality was performed in patients who are on chronic hemodialysis and who have ischemic steal syndrome at the background of arteriovenous fistula (n-56), heart failure, (n=74), as well as in patients without complications of permanent vascular access (n=88) - that was a control group. Method SF-36 was for the first time used for these in order to make objective assessment of life quality. On comparative analysis of results of patients’ questionnaires it was found physical functioning in patients with ischemic steal syndrome by statistic was reliably lower than in patients of the control group. After hemodynamic correction of the arteriovenous fistula this index became much better. Role functioning in cases with ischemic steal syndrome was reliably lower than in patients of the control group. Psychoemotional reaction on the absence of pains in a rest state was more marked in patients who underwent ischemic steal syndrome. Fear for the return of pain syndrome does not allow these patients to assess high their own common health state, although the index of their life viability is actually the same as in the control group. Improvement of social and emotional functioning was observed as well as psychologic health state. Physical functioning in patients with heart failure by statistic was reliably lower than in patients of the control group. After hemodynamic correction of the vascular access this index became much better, although it doest not reach control values. Role functioning in cases with heart failure was reliably lower than in the control group but after correction of the vascular access the patients limit themselves less. Psychologic component that reflects the absence of prospects to get out of the neccesity to have chronic hemodialysis do not allow patients with heart failure to value high their common health state After a reduction of manifestations of heart failure it was noted a reliable improvements in social and emotional functioning although psycologic health state was increased insufficiently. Correction of hemodynamic disorders in the examined patients sufficiently improves quality of their life.
