Some clinical and immunological aspects in patients with diabetic foot syndrome

The diagnosis was established in accordance with clinical and laboratory research conducted of 29 patients, who were 30-80 years old and had diabetic foot syndrome. The study of the dependence of changes in the concentration of cytokines from nature of inflammatory process has shown that the level of interleukins-1 and -6 increases with the severity of the inflammatory process in patients with diabetic foot syndrome was significantly higher than that associated with the severity of the inflammatory process. The level of interleukin-8 is reduced in the diabetic foot syndrome due to changes in the adhesive properties of neutrophils, cell migration and adhesion at the site of entry of the microorganism. The level of IgA and IgG can be indicative of the presence of secondary immunodeficiency. The first group of patients also have shown the increased concentrations of tumor necrosis factor, which is associated with the activity of the inflammatory process. In the group with adverse prognostic course of the process there were high rates of IgG, mterleukin-4 and-6. It was established that the deeper tissue damage, the higher the value of the soluble receptor of tumor necrosis factor is. With effective treatment, levels of Il-6 have decreased significantly. The data obtained indicates the possible employment of determining of level of cytokines and their dynamics for predictive evaluation of the effectiveness of therapeutic interventions.
