The aim of the study was to carry out comparative investigation of the vasomotor and metabolic functions of the vascular endothelium and the level determination of the endothelial dysfunction in patients of different ages with the I-level arterial hypertension. 98 young patients with the I-level arterial hypertension, with the 0, 1, 2 risk (middle age 72,2±19,2±3,3) and 30 elderly patients with the I-level arterial hypertension, with the 2, 3 risk (middle age 72,2±2,7) were included into the investigation. The control groups included practically healthy young people (44 persons, middle age 19,4±3,5) and men (45 persons) of 65 and older (middle age 68,6±2,5) with the normal arterial hypertension. The disorder of the endothelial-based vasodilation both in young and old patients with the I-level arterial hypertension is accompanied by the high reduction in the production of nitrogen oxide by endothelium. In this case the level of its production in young hypertensive patients is 36,0% higher than in elderly patients. The intensity of the endothelial dysfunction of vessels in patients with the I-level arterial hypertension and of different age is in close correlation with the indications of disbalance between pro- and antioxidant blood activity appeared in raising the methane-dicarbonic dialdehyde concentration in the blood serum and the index decrease of antioxidant activity/ methane-dicarbonic dialdehyde in comparison with the groups of healthy people in answer to the strengthening of the pathologic vasoconstriction of vessels. It's estimated that hypertension both in young and elderly hypertensive patients is accompanied by the progressive endothelium dysfunction of vessels (in elderly patients it is more evident). This occurs due to the reduction of the production of nitrogen oxide by endothelium and disbalance of the system prooxidants - antioxidants in favor of the 1st ones and this lead to the remodeling of vessels.
