In this retrospective study, we analyzed mortality among patients with high-risk prostate cancer treated in Samara Regional Clinical Oncology Dispensary. We assessed the efficacy of four treatment strategies: surgery plus external beam radiotherapy, surgery alone, external beam radiotherapy plus conventional radiotherapy, and high-intensity focused ultrasound (HIFU) therapy plus conventional radiotherapy. In patients with high-risk prostate cancer who underwent surgery and conventional radiotherapy, the 8-year relapse-free survival rate was 80% and overall survival rate was 82%. We observed no significant differences in 5-year survival between patients receiving surgical treatment and adjuvant radiotherapy and patients who had surgery alone. However, the 8-year survival was better in individuals who underwent surgery followed by radiotherapy than in those who had surgery only. Surgical treatment as monotherapy en sured significantly better long-term survival and overall survival than HIFU therapy plus hormone therapy and radiotherapy plus hormone therapy.
