We present a case of diagnosis and treatment of 48 years old patient with a giant tumor of the anterior mediastinum. During 10 years before admission to the hospital the patient had not attend preventive examination. CT scan of the chest revealed a tumor with clear smooth contours, dimensions 21 x 13 x 24 cm. At operation it was found out that the tumor is closely fused with the diaphragm, the dome of the pleural cavity. On its back surface it grows in the lung tissue, sprouting vessels of upper and lower lobe of the lung, making it impossible to remove the tumor without lung. Compression of lung tissue is registered, which is not involved in breathing. Pneumonectomy and removal of the tumor was performed. The postoperative period was unremarkable. For early diagnosis and treatment of such tumors radiography of the chest one time a year is necessary.
