Лучевой лоскут предплечья для реконструкции тотальных и субтотальных дефектов носа

Статья посвящена основным аспектам использования свободного лучевого лоскута предплечья в реконструкции наружного носа. В случае тотальных и субтотальных дефектов наружного носа объема локальных и регионарных лоскутов может быть недостаточно. Кроме того, местные ткани, которые подвергались лучевой терапии, в случае злокачественных новообразований носа, являются неподходящим пластическим материалом, так как нарушается их кровоснабжение и снижается эластичность кожи. В этих ситуациях три основных компонента носа - слизистая оболочка, опорный каркас и наружная кожа - могут потребовать реконструкции с использованием тканей, взятых из отдаленных участков путем пересадки свободных микрососудистых лоскутов. Лучевой лоскут предплечья достаточно тонкий (позволяет реконструировать нежную кожу носа), пластичный (позволяет скорректировать форму носа и при необходимости сформировать свободный аутотрансплантат), относительно прост в заборе (имеет постоянную сосудистую анатомию), а также хорошо васкуляризирован, что делает его лоскутом выбора для пересадки на ранее скомпрометированное реципиентное ложе.
