Our experience of treatment of diffuse purelent peritonitis in children

The purpose of the study. Improvment the treatment of diffuse purulent peritonitis with using of therapy with antihypoxant Reamberin and hepatoprotector Remaxol, nutritional support, laparoscopic sanation of abdominal cavity. Materials. 232 children aged from 1 to 15 years with diffuse purulent peritonitis were treated at the children’s surgical departments of Samara from 2001 to 2014.The general group included 148 patients who were applied the optimized pathogenetic therapy. The study group was divided into two subgroups: 66 patients in the pathogenetic therapy who were treated with antihypoxant Reamberin, and 87 children who were treated with Reamberin and hepatoprotector Remaxol. All the children from the general group received nutritional support (trophic feedings); abdominal laparoscopic sanation was used in the surgical treatment. Complete examination included the study of the dynamics in the level of white blood cells, leukocyte index on Kalf-Caliph, erythrocyte sedimentation rate (ESR), temperature, total albumin concentration, transaminase levels. Results: Comparison of the studied parameters in the general and control groups showed more rapid decrease in the symptoms and signs of intoxication (leukocytosis, LII, body temperature), relief of enteral insufficiency, recovery of protein-synthetic function of the liver, a decrease of cytolytic and mesenchymal-inflammatory syndrome in the study group, especially in the subgroup with therapy of Remaxol. Conclusions: optimization of treatment of diffuse purulent peritonitis with the administration of the combined drugs - antihypoxant Reamberin, hepatoprotector Remaxol, nutritional support and implementation of laparoscopic abdominal sanitation led to improvement in the results of the therapy.
