A new method to predict the course of acute pancreatitis on the base hepatodepression markers (prospective study)

Purpose of the study: To develop a method for predicting acute pancreatitis based on signs of hepatodepression.Methods of research: The clinical and laboratory studies of patients with acute pancreatitis varying severity (n = 51) tested blood test with the determination of urea content, bilirubin, creatinine, albumin with assessment its effective concentration and calculating an index of toxicity. We evaluated activity of aminotransferase, alpha-amylase, phospholipase A2, superoxide dismutase, determined the content of middle mass toxic peptides, content of products of lipid peroxidation. The obtained data were processed by variational statistics methods.Results: Dynamic changes of the determined indicators are estimated. The study revealed indices, differing the greatest reactivity and high stability of changes in conjugation with the severity of acute pancreatitis. These include indicators of endogenous intoxication, oxidative stress markers, activity of phospholipase systems and alpha-amylase. All of it connected with liver function impairment. They also served as the basis for the design of a prognostic index. We have elaborated a formula and found the reference values of the index of predicting the severity of acute pancreatitis.The conclusion: Determining the prediction of acute pancreatitis severity in-dex gives an indication of the direction (progress or regression) of the disease course and evaluation the effectiveness of treatment over time.
