Features of the Clinical Course of Shigellosis with Different Species of the Pathogen in Modern Conditions
The article presents the results of an epidemiological analysis of shigellosis incidence among the urban and rural population of the Kyrgyz Republic for the period 2013–2025. The distribution patterns of various Shigella species and their impact on the clinical course of the disease were studied. The aim of the study was to analyze the prevalence of Shigella flexneri, Shigella sonnei, Shigella boydii, and Shigella newcastle in patients with shigellosis hospitalized in infectious disease hospitals and to determine the impact of individual species on clinical manifestations and the development of complications. A total of 584 patients with suspected acute shigellosis were observed, of whom 517 (88.5%) had the diagnosis confirmed bacteriologically. Shigella flexneri was the most frequently isolated bacterium, occurring in 312 (60.3%) patients, followed by Shigella boydii (92) (17.8%), Shigella sonnei (81) (15.7%), and Shigella newcastle (32) (6.2%). Males predominated among the examined patients (64.3%), and most patients were aged 15–40 years. It was established that infection caused by Shigella flexneri was characterized by a more severe clinical course, accompanied by a higher frequency of complications, and was the main etiologic factor in severe forms of shigellosis. At the same time, dysentery caused by Shigella sonnei often had a mild course. Diseases caused by Shigella boydii and Shigella newcastle often had an insidious or atypical course, occupying an intermediate position between infections caused by Shigella flexneri and Shigella sonnei, which hampered timely clinical diagnosis. The obtained results indicate the dominant role of Shigella flexneri among shigellosis pathogens in the Kyrgyz Republic and confirm the need to consider the pathogen species when predicting disease severity, the development of complications, and the selection of treatment and prophylactic strategies.
