Peculiarities of virulent pathogens in diabetic foot infections

The aim of this study deals with investigation of virulent pathogens peculiarities in diabetic foot infections and choice of empirical antibiotic regimens. We have analyzed wound culture and sensitivity results of 152 inpatients who had required surgery for the first time (group 1) and results of 49 inpatients who have been previously hospitalized and operated for diabetic foot infection (group 2). Staphylococcus aureus was the most frequently isolated and virulent pathogen. MRSA was isolated from 13,5% specimen of St. aureus in group 1 patients and 39,9% in group 2 patients. Acinetobacter baumanii was isolated in 20,5% cases, Enterococcus faecalis in 16%, Pseudomonas aeruginosae in 13,6% specimens of patients without previous history of diabetic foot infection, and more frequently isolated from patients who have been previously hospitalized. Initial regimens for inpatients who had been operated for the first time should be broader spectrum and cover the most common pathogens (Gram-positive cocci). Consider adding an agent active against MRSA and Gram-negative bacilli if the patient had been hospitalized and operated previously.
