Long-term results of different methods of surgical revascularisation of the femoral-popliteal occlusions

The study evaluated the results of surgical treatment in patients with obliterating atherosclerosis of lower limb arteries in early and remote postoperative period with occlusion of the femoral-popliteal segment. Operated 308 patients with chronic ischemia of lower limbs undergoing surgical treatment at the University Surgical Clinic of city hospital them. D.D. Pletnev in Moscow in the period from 2001 to 2017. Surgical treatment was performed in a volume of femoral-distal popliteal or femorotibial bypass. As the shunt material was used autovein and synthetic prosthesis. To improve the work period of the synthetic graft and limb salvage, some patients used the methods of autovenous plasty of distal anastomosis or of the use of a composite prosthesis (prosthesis + autovein). Some patients were also performed hybrid intervention - open and endovascular procedures in the same operative session. Long-term results of shunt patency and limb salvage is estimated for 5 years, with Kaplan-Meier method used for statistical processing. Our data show that vascular and hybrid surgery, along with methods of plasty of distal anastomosis in cases of synthetic prosthesis usage would extend the indication for open reconstructive surgery and prolong the time of function of the c graft and improve limb salvage.
