The results of two different approach to law obstructive malignant jaundice treatment are demonstrated. In the main group (n = 96) mostly performed surgical decompression. Choice of operative method was choledochodigestive anastomosis. In the comparison group (n = 128) widely used minimally invasive intervention and cholecystodigestive anastomosis. The best long-term results in the main group with the similar procedure related morbidity and mortality in both groups is a serious argument in favor of surgical decompression. Among surgical procedures more durable palliation of jaundice provides bypass to the bile duct.
