Периферические биологические факторы и биомаркеры суицида

При обсуждении биологических факторов и патофизиологических механизмов суицидального поведения нельзя ограничиваться только анализом процессов в ЦНС. Как свидетельствуют многочисленные данные, в патологические цепочки вовлекаются разнообразные периферические процессы - метаболические системы организма (в частности, периферические серотонин, триптофан, нейрокинуренины, норепинефрин и др.), липидный обмен, иммунитет (в частности, система цитокинов), нейрогормональные реакции, а также кишечная микрофлора как компонент оси «кишечник-мозг». Все они, а также их генетические маркеры, в конечном итоге оказываются так или иначе связанными с суицидальным поведением - от суицидальных мыслей до завершенного суицида. Что при этом является причиной, а что следствием - дифференцировать довольно затруднительно. Скорее всего, эти связи многосторонние и носят сетевой характер. Интегрирующим механизмом, наиболее вероятно, является стресс-реакция, охватывающая весь организм и затрагивающая физиологические процессы, сознание, поведение и мышление. Это имеет значение одновременно и для понимания патофизиологии суицидальности, и для поиска периферических маркеров, которые могли бы быть использованы для уточнения риска, предикции и превенции самоубийств. Наиболее перспективным является комбинирование социальных, психологических, биохимических и генетических тестов, и совершенствование стратегий, аналогичных конвергентной функциональной геномике.
